This Privacy Statement applies to all personal data that Fit-therapy processes from its customers.

If you become a customer of Fit-therapy or if you provide personal data to Fit-therapy for any other reason, you give explicit permission to process your personal data in accordance with this Privacy Statement.

-Your first and last name

-Your address details (possibly postal address if necessary), telephone number and e-mail address

-Your intake form and/or anamnesis with relevant (health) data and date of birth

-Notes of your treatment(s)

-Your photo (from, for example, social media, whatsapp)

2.2. Fit-therapy processes the personal data referred to in sub 2.1 for the following purposes:

-Your first and last name will be used for contact about appointments and the possible cancellation thereof

-Your address details (possibly postal address if necessary), telephone number and e-mail address are used for making appointments, sending invoices, reminders, invitations and information about services

-Your intake form and/or anamnesis with relevant (health) data and date of birth are used to make a treatment plan and to monitor the progress of the treatment(s).

-Notes of your treatment(s) are used to monitor the progress of the treatment(s).

-Your photo will be saved for recognition

Email notification:

Fit-therapy can use your name and e-mail address to provide you with its e-mail newsletter with information about activities, services and other interesting information. You can unsubscribe from these mailings at any time via the unsubscribe link at the bottom of the mailing.

Retention periods

3. Fit-therapy processes and stores your personal data until you request that the data be deleted. Subsequently, your personal data will be destroyed.

4. Security measures and processors/processors

4.1 Fit-therapy has taken appropriate technical and organizational measures to protect your personal data.

4.2 Fit-therapy does not use the services of third parties for the processing of your personal data. Fit-therapy is therefore the so-called processor itself. Fit-therapy uses an online accounting package for processing the financial data.

5. Right of inspection, removal and questions about complaints

5.1 You can submit a request via the owners of Fit-therapy to view, receive, change or delete your personal data. Fit-therapy will process your request and inform you within one month of receipt.
